ABSTRACT

OBJECTIVE@#To establish a new animal model of grading skeletal muscle contusions that could be controllable and repetitive.@*METHODS@#The rats' gastrocnemius was injured by a new weight-dropping device designed. The force acting on gastrocnemius with a comparatively constant duration and inducing elastic deformation of the gastrocnemius was expressed with velocity (v) and deformation (DF). Instant velocity was changed to create gastrocnemius contusions. Pathological changes of gastrocnemius were graded by the gross and histological examinations of 39 rats.@*RESULTS@#At low level of impact (v: 2 m/s, DF: 5.5 mm), mild injuries were detected in epimysium and superficial layer of gastrocnemius. At moderate level of impact (v: 2.5 m/s, DF: 6.5 mm), the injuries were observed in epimysium and whole gastrocnemius. At high level of impact (v: 3 m/s, DF: 7.5 mm), severe injuries were seen deeper to soleus with more extensive skeletal muscle damage.@*CONCLUSION@#Grading of skeletal muscle blunt force contusion is created by parameter of velocity and muscle deformation. The model could be used for further research on skeletal muscle contusions.

ABSTRACT

Se presenta un caso de la Unidad de Cuidados Intensivos Pediátricos del Hospital Militar Central, quien presenta insuficiencia respiratoria secundaria a atelectasias pulmonares migratorias masivas. Antecedente de trauma automovilístico 20 días antes por lo que permaneció 24 horas en observación en otra institución sin evidencia de alteración. Al ingreso además del cuadro respiratorio presenta cambios neurológicos consistentes en hemiparesia izquierda e hipotonia generalizada que evolucionan hacia cuadriparesia espástica, TAC cerebral y de columna cervical normales. RMN cerebral normal y de columna cervical muestra cambios compatibles con contusión medular a nivel de C5 Diagnóstico de síndrome de injuria medular sin alteración radiográfica (Sciwora) y atelectasias pulmonares secundarias.

ABSTRACT

No período de 15 anos, 18.095 pacientes foram cadastrados na clínica privada, Centro Médico Esportivo Säo Francisco da Penitênciaria e no Serviço Médico do Fluminense Football Club. Observamos que 10.719 (59,23 por cento) pacientes eram portadores de lesöes esportivas sendo que 2.670 (24 por cento) correspondiam a lesöes musculares. Apresentamos uma classificaçäo analisada criteriosamente, baseada na observaçäo clínica evolutiva de atletas de alto nível, com um minucioso registro diário, cumprindo cada etapa terapêutica até sua total rebilitaçäo
